Brooks Automation is committed to the safety of our customers and our employees. As such we have health and safety documents that must be completed with returns and/or destructions of CTI and Granville-Phillips products. This page contains the documents or links to the databases. Health And Safety Certificate
The Health and Safety certificate is for any CTI customer who wishes to return their pump to Brooks Automation and needs an RMA number. Before RMA numbers can be distributed the certificate must be complete as per ISO regulations.

Certificate of Destruction
Brooks Automation realizes that customers may wish to dispose of their pumps themselves. Before doing so, Brooks Automation requires that the serial number tag be returned to Brooks so we can properly track serial numbers. This is the document to complete.

Granville-Phillips Hazardous Material
The Hazardous Material Exposure Disclosure Form must be completed when any Granville-Phillips product is sent back to Brooks Automation.
